Melanie R. Weltman

Research Assistant Professor

Dr. Weltman's clinical and research interest is centered around optimizing medication therapy for patients with chronic kidney disease. She joined the faculty as a Research Assistant Professor in the Department of Pharmacy and Therapeutics in 2024. She is core faculty in the Center for Population Health Management in the Renal-Electrolyte Division at the University of Pittsburgh School of Medicine.

Dr. Weltman received her Doctor of Pharmacy degree from the University of Pittsburgh School of Pharmacy. Following graduation, she completed postdoctoral work at the University of Pittsburgh as a clinical research fellow. In this role, she was key in the implementation of the Kidney Coordinated HeAlth Management Partnership (Kidney CHAMP) study, which tested the effectiveness of a multifaceted electronic health record-based intervention on the delivery of evidence-based CKD care. Dr. Weltman has used Kidney CHAMP data to explore medication-related outcomes including frequency of medication therapy problems, acceptance of recommendations for newer cardiorenal-related medications, and use of guideline-recommended medication therapy.

In 2022, Dr. Weltman joined UPMC as a Clinical Pharmacist to provide medication management services to patients with high-risk chronic kidney disease through the UPMC Health Plan sponsored CKD e-consult program. She enjoys working with multidisciplinary teams to deliver patient-centered care
